I haven't played a lot, but from what you have mentioned, the only thing I did was drink too improve relations. But none of those is mandatory, you can turn off smoking, and I have never had to fight people in the game. Voyeurism is the one thing that may happen, but I'm pretty sure you have to initiate it, so honestly, you should be fine.
