If the Mod adds things such as "Items in the Convenient Store or the Pharmacy Store", those Items will immediately appear in the Shop after you install the Mod.

But if the Mod adds things such as "Clothes, Bot parts, or Personality Module", those will start randomly appearing in the Shop after a few in-game days. And for "Modded Clothes in Annalie's Shop", you can immediately order every Modded Clothes after you install the Mod.

That makes sense, but why do I sometimes start with two legs, or two arms? It changes every time I would start a new profile. Is it meant to be random? (also is there a way for me to start with more stuff?)

The Parts that you get from the start is Randomized. Currently, there is no way to start a new save file with more stuff other than Both Arms or Both Legs and there is not even a single Mod for this. But you could use some Cheat instead by going to Browser on Anon's PC and searching for "iamacheater" in the Browser and you can get some start money from there.
