yes, same happened to me. I tried everything but got no luck. I think the puzzle at the beginning has something to do with the witch appearing or not. Not sure....
Try to solve all puzzle in the first room for the black monster to go away. Make sure the second player is giving you all the right answer. Once all puzzle in the first room is done, try opening the door where the black monster located.
