Got a couple myself:
Head goes behind top fences in the farm home area.
Tommy's head is cut in half by the bottom of the rat-infested ranch in childhood memory cutscene when lockpicking.
Chest in bottom left of docks at the start of the game can be looted through the fence without walking around to it.
Rat skin from childhood memory is dropped and stays in inventory. [long time to keep a rat skin]
