What is the best gear I can find for each slot ? and till now I haven't found any underwear yet, is that normal ?
I don't think you can find any underwear. But it can be bought from Rose in Bareshade.
Personally I always play with:

Might not be the best items, but it fits my playstyle. Just try to find something that you personally enjoy playing, since the gear can more than make up for other stats that you might be leveling up.
A couple of questions for this.
first, what are the Specter's Shoes, and how do you get them, they seem interesting?
second, What's with the inconsistency with getting the harness? it seems like every other play through I get the option after I defeat Barrel the first time, but i con never get it after? Is there some hidden requirement in the first fight that I'm missing?
