What you need is a License Agreement written in a note or doc, you can find these easy in the internet. This is an example: https://legaltemplates.net/form/licensing-agreement/

Thanks for the reply.
But CC BY-ND 4.0 contradicts 2 of my requirements:

Redistribution (I do not allow it).
Derivatives (I allow it.)

I have not seen that LA template yet. Imma check it out, thanks!
