Can I ask what hardware you were using? Also, were you standing and physically moving around or staying still and using joystick?

I ve been playing on the Valve Index.It happend twice when I was moving with the joystick and I think I tried grabing the grenade with both hands at that time.
Ahhh, I think what it is is that the grenade for some reason collides with the playerbox - Thanks for reminding me to fix that!
Yea that is a common problem. Happens every now and than :)
