Yes but some of them never replace the previous one. For exemple, "Natural Pride" doesn't appear even though I managed to get "Being naked is embarrassing.." to lvl 10. Same with "I Fucking Need It".

Yes once you get the first 4 Thoughts to lvl 10, it doesn't unlock the next ones so you're basically soft-locked. I had to edit my save multiple times to see the end of the game. The dev is working on an update so let's just wait
