Do you remember where it happened :)?
on many places, but the one i remember is:
when I was doing the level with the gear I just circled around it wich caused the hand to get stuck and glitch in there without being able to return to the base, thus continue the level. A fix would be automaticaly changing screen after some time to avoid this
