Do you have any other pages you'd like to be included as links when credited? I'm considering using one or more tracks in an art video for youtube, so it'd be for the video description. In the video itself I'd probably just name the tracks and your name, e.g. "now playing: 'Port Town' by Chris Logsdon" :)
