Maybe random i made is too harsh or you were unlucky.

Dandelion - White World random event (In the beginning route there is a small field without anything. The statue will appear)

Lily - Town Apartments random event (Before elevator room.  When you go up stairs instead of elevator you will found yourself in green room)

I don't quite get what you mean @_@ It's 13 effects that you can find through the dream and use them from inventory. All 13 effects are available in actual and previous versions.

Maybe you reference to fused effects? We don't have them. All effects are separate items.

Only one effect need other effect to be found. Different effects can require special actions (like solving puzzles) or not.

We didn't have many testers to catch graphical bugs. Thank you for reporting them i will fix them in next update.

Hi! Thank you for  your feedback. I fixed bug. The issue was that we updated beginning cutscene, but forget to update the effect itself.
