How does the game decide where you can and can't place cards? I've had it force me to play on a diagonal rather than fill out a horizontal path with the same amount of cards

Lot of pixel dungeon inspiration huh

They aren't really that hard to punish, just block knife and hit them with some lights or crouching heavy. You can also play campy with the gun.
