You need to get her pleasure up above the little indicator but below full, while it's there orgasm strength builds up. The various Tease actions are helpful for that.
That's an element of the game's ending, depending on your stats of course :)
You need to let her get used to it on her own terms. (bj->warn her a few times, you'll get different reactions as her comfort progresses).
