Thanks. How do you get Bernard's 4th heart? Is it someting at the fair too? Does Xer, Rhot or Roushk have a 2nd or 3rd heart?

How do you gain Caleb's and Rhot's first heart?

You need to go hunting with him for the 1st heart, go to the hot spring for the second, and for the third he needs to invite you over to his house iirc. You'll need to ask about his friend after finding the journal in the hut after starting the lost friend quest and ask about hunting to go hunting with him. You need to go on Friday between 18:00 and 0:00. To go to the hot spring you need at least 1 heart and ask about hot springs.

Which werewolf? The generic ones or Bernard/Logan?

How do you get Caleb's first heart and Bernard's 4th heart? And how do you do the shower scene with Caleb?

Can you free them? Also what happens if you imprison them?

What happens if you deny the cure/wolfsbane to the person who you rescued?

How do encounter the werewolf alpha?  I assume it's in the wolces den at night but is it a random encounter with the skulls or do you have to go to a specific tile to encounter him?
