Thank you for the feedback! When I was developing the game, I wanted it to have an arcade-esque difficulty and a  'one more try' feel, so that's why I didn't implement lives. As for the context, I wanted players to be able to imagine things like why the Heart Stopper is chasing them, but I do think adding a bit more context is a good idea.

Thank you for the comment! When the Heart Stopper is about to get you, you can go off-screen and come back at the other end of the line. This can also help you reach the power ups before the Heart Stopper does, but they are deliberately placed randomly in a rectangle near the centre of the screen so they're hard to get. To be honest, I think I should have added this screen-loop mechanic to the How To Play text.
