Yeah you have to pay 5$ to get the NSFW version of the game,  the notification that appears after clicking the download button shows all the included files and if you click the one that has the nsfw name instead of 3$ for support it must say 5$ then after you pay you´ll be able to download the nsfw version. And yeah, it´s a one time payment if  you delete the game or want it in another device just go to your library in itch and the game will appear in "things you own", when the game updates you have to download it again in the main page of the game that at the top will say "You own this game" and download it from there 

All I wanted to hear with the new voice patch was Hikaru to say: `Shooting you right in the heart, it´s Hikaru!´  TT_TT
