I assigned all bones but the biped looks completely off when generated. What could be the issue here?

Somehow you assigned one of the right side helpers to the left. I circled it here in this image:
As for the head, the position of you head helper will help with that.
Let me know if that works for you.
I fixed the helper but still got the same issue..
There should be a finger helper 0. You have finger helper r 1 assigned multiple times which is most likely causing you issues. If you are still having issues, feel free to attach the .fbx file here or email it to support@mattrifiedgames.com
