Intriguing concept, looked very polished too.
Great idea and visuals, I loved some of the things pulled up and really fun audio work with some of the things as well. I feel like there could have been a bit more context around what you're doing and the actual consequences of the form you're filling out. I couldn't really figure out why I would say yes or no on keeping an item for instance. Also the text on the forms is very blurred, at least for me, and it made it impossible to read the paragraph on the top of the form. I love the vibe your game brings though. Great work!
