Extra chaos was my favorite mode! But it comes with a paradox as it's a bit more easy to survive by just turning on yourself. The idea of the big overload growing is nice but it's masking one part of the screen that could lead to some frustration, I don't know if it was intended :)
In normal mode, you can't shoot directly after an overload on the opposite as the chaos mode.
The art made me think about binding of isaac, super meatboy, that's well known code and that works perfectly with you mechanics and ambiance :)
A tips here, if you don't know, is that you can use .toString("00") to display 2 digits (for your timer) even if the current time is 3 sec.
I would've like to see my final timer btw once the game is over! That said, it was fun to have a couple of run :p Congrats!

The graphics are overall nice, the enemy design is particulary unsettling. Can't say the same about the background, mostly because it has a different resolution than the rest of the art.
The gameplay is simple and kinda fun. The shooting outside of the inital shots lacks audio-feedback and thus feels rather weak. The death screen also comes in a bit to abrupt and takes too long, but is otherwise neat
It quickly becomes chaotic with all the enemies, and as such fits the limitation really well.

Nice game! I really liked that you added a chaos+ mode, it was really fun to just be overpowered for a bit, and it was similarly nice to see a cleaner version too; nice to see the game is considerate like that. I liked how the difficulty ramped up too, it kept things challenging, and the hordes were definitely chaotic! Charging up that big shot was a neat mechanic too, I liked it! Well done on making this game.
