If I change the option of a choice on the next playthrough, does it count as 1 choice or 2?

E.g: On the first playthrough, you have 1 choice, ask question A or question B.

On the second playthrough, you have 1 choice, ask question C or question D.

Is it 1 choice or 2 choice?

Alternatively, if I make options in the choice which can lock and unlock depending on which playthrough they are on, is it allowed ?
