This is less of a bug and instead a Soft lock I discovered by accident.
When collecting the Radioactive Capsule in story mode, the radiation damage was faster than normal after having died to it and reloaded just before hand. I panicked, not wanting to go through the hassle of lost progress and redoing it yet again, and saved right when my screen turned red and I collapsed. That save is now effectively soft-locked, as I just die and get sent back to the title screen every time I load it.
