Then you can get the game on Steam instead? The dlc itself is a free version of the game after you purchase it there. If you already happened to buy it on itch, you can run it through steam as a non-steam game. 

If you're worried about people seeing your steam activity, Steam has functionality to hide games so nobody will see you play/own them.

My problem is that in my country such 18+ content is blocked on Steam. I can activate external keys and the like, but not free DLCs directly. That's why I'm asking.

I couldn't unload the dlc separately. There's not only artwork in there, but there are also some code changes, so in Steam when you load the 18+ DLCs, it's essentially loading a new build of the game
