Tested on Valve Index. Open steamVr first than start this program out of the explorer.

Tried it again. Don't know what you mean by explorer BlueEdgeCrazy but it still wont work for me.

Itch.io has the sandbox mode as standard activated. A lot of VR games don't connect with steamvr or occulus in this mode.
With right click on the game and click on open folder in the menu -> itch.io navigates to the installtion directory of the game with the file explorer of windows.
Open the game with click on the exe e.g.  \itch\apps\physical-checkup-vr\Physical Checkup VR\Physical Checkup VR.exe

This works and it's a little bit safer than deactivating the sandbox mode for all games.
You can scan the exe-file with a antivirus tool to be a lit bit more on the safe side.

Oh, you mean in the app. No I download all games straight from the itch.io website and start the .exe files straight from the download folder.

I also just tried downloading the app and trying the way you did it and it still just showed up black with a clicking noise happening if I looked in the right direction.

Maybe Microsoft's Dependency Walker tool can help you. It tries to find the missing DLLs and other things by searching the program.

Alright so for me it's more of I just can't seem to find the .exe it's just the Physical Checkup Vr with the Icon App Game Kit if you did explain it could you please go through it again because i may have gotten lost somewhere. Thank you but I'll also keep re reading through this to see if I can find a wa to work it that way.

I tried The dependency walker tool and it gave me a lot of errors when I had it scan physical checkup. So I think the game is just straight up broken
