If you're curious why I used a liquid in my first attempt, the game never said wards couldn't use liquids, and I felt that soaking a woven charm and letting it dry made more sense than covering it in powder.

Ah, I think what may have been going wonky here was a rule that was put into the game but had not been added to the herbalists notes, which stated that wards had to be made with leaf first, followed by liquid then powder. Thanks for pointing this out, it's now been fixed :)

That still doesn't explain why my second attempt, using two leaves and a powder failed…

Ahhh okay I see. Another rule was transcribed incorrectly - it should be lowest acidity first when there is more than 1 of the same type of ingredient, but the book states it the other way round. Those three ingredients are correct however. Apologies, the 2 things to be made are randomly picked from a larger list and it seems you got the only one that uses this rule!

That explains a lot…

I also looked at the list of ingredients recently, and unless the ingredients have changed, my first attempt would've failed anyway, even if I put the liquid last and used goldenflower first, due to me misreading something as (leaf, 1, 2) [didn't exist in my recent read through]

But if what you say is true, that means only one ward exists. If do, I think you should add more, and adjust any rules that prevent adding additional wards. But that's just my opinion.
