It looks and sounds great, I just can't figure out how to kill granny even after throwing the bottle.

If you don't encounter a bug, when you throw the bottle your target come towards it and when that happens, you can crouch (with CTRL) and come to its back to kill it (with Left Click).If you are running to close to the target, it will detect you.
