that's the mouth fiend, Lura. You need to be mean to the brothel girs twice. You do this by asking for a blowjob and then fuck the shit out of them (only works with the first 2 girls and not the 3rd , i think).
You need to go the town square after that and select eavesdrop conversations, and Lura will appear immediately.
