I have checked and I do not see it when I ctrl-f through the responses.
I would like to request you resubmit just to be safe. Also previously did you get a copy of your google form entry mailed to you?

Just resent it! If it brings any comfort, I think it might've been an issue on my end. If I had to guess, I think my touch screen didn't properly register my tap on the "submit" button the first time, or either that my internet connection was acting up and I just didn't notice lol. With that said, my form should be in now. I'll also be emailing you a copy of the form I just sent just to be on the safe side. :]
