So... what's the general structure of updates to this game? Like does each 0.0.1 involve a new guest to the spa, each 0.1.0 is a main quest with branches, and something like that?

Every new build will at add at least 1 quest to the game. The 1.0.0 release added the "finale" quest which has credits roll after it. The 1.1.0 update included a major layout change as the sauna was reconfigured. I anticipate most if not all future updates will just increment the last digit as new quests are added: 1.1.12, 1.1.13 and so on. -Oz
