I was counting the tiles, so I'm sure I was supposed to survive. 5HP and 4 tiles destroyed so I should be left with that 1HP. No idea what triggered that.
Simalar thing has happened to me, although not in the order of events you described. It was a couple of patches ago so it might have been fixed but if you use vigors brew as you are meant to die you will die but still have hp left.
