It's Irene. It happens after Regina beat her up. She must be recruit and go to Shepherd's household. Keep her assist option to no so she will not rescue you from ghosts. Catch the ghost, look for two bells in the map. Cut it down with dagger. Wait on second floor for 10 seconds then go to dojo.

you will see a small flame orb floating randomly in some of the rooms, if the flame orb is red, wait until she starts chasing. You must stab her with the Cursed Dagger AFTER SHE STARTS
MOVING. After stabbing her, just let her go. The next time she hunts you, just let her capture you, this will trigger the special animation.

Cause it is pattern for other event. You need to play around with those symbols. I dont think there`s clue in the game yet about how to unlock it, but you definetly can. Its how I did it
