I'm almost finished with my game.  can I have an example of what it means to "put my name on my assets"? do I have to go back through all my assets and put my name on them? 

I think it just means putting your name in the credits section, but i am not sure.

You need to credit everything in the game, meaning a credits screen (ideally on or accessible from the title)
