I can't help but always pick the nice options for the kid haha I like the multiple choice aspect of it, but honestly I would only pick the obvious wrong choices just to see what happens, maybe having more of a gray area so that it's not so obvious or having to manage resources like money (Kinda how they do in Papers Please, where you get only certain amount of money and have to pick between food, medicine and heating to keep everyone happy)

that's a great idea, i like making narrative games with karma concept. maybe i'll add that idea in my game with more complex story than this.
