I can imagine pregnancy is tough to implement in a game, probably why so many have them as an ending only option type thing. Could take inspiration from games like Lilith's Throne and have offspring being taken away to a vague good life away from the MC, or you could maybe do a phantom pregnancy thing, where the MC adn others are "pregnant" with all the symptoms, but aren't actually pregnant for real. Feels like it could fit well with the expansion systems?
Either way, tomboy/goth/preg supremacy.
