Your game is fun, the music is ok and goes well with the rest of the game.
The art is nice but could have used a bit more definition, using more contrasts or borders maybe, to make things easier to see. A good example would be the obstacles.
Also I was under the impression that I didn't "die" even with 0 heart, maybe at -1 or -2, not sure.
I like the fact that we can see the cars that are outside of the screen, that's a nice touch!
About the pause, maybe it could have been more present, by reducing the cooldown. The pause didn't seem that useful to me.
The car's movement controls were enjoyable, but I didn't find the pause key convenient, maybe that's because of my keyboard though, it's not a native qwerty.
Overall it is a nice piece of work, good job!
