I don't understand how not working consecutively works

While you are not working your are thinking. Does that count in the timer? 

Also, let's say you need someone to playtest your game and have nothing to work on anymore. Does that time count in the timer? 

Should I not count when I am sleeping or taking breaks?

You should work that means code and make assets  to complete the game in 48 hours, sleeping and playtesting does not count

I wouldn't do that but isn't it ok according to the rules to look at the art kit when the jam starts and then wait for a couple of days and think about it?

I would count playtesting into 48 hours. 

As I have stated in other topic - I think it's on your own morale and honor system. I will run the clock whenever I have any tool for game development opened (f. e. Google sheets, Audacity, Godot, etc.)

48 actual hours of work during the 9 days  this Jam will last is more than 5 hours a day on average. For the ones that have a full-time Job and/or studying or doing other stuff is not going to be easy to maintain that rhythm anyway and they probably won't spend them all.
At least on my case that I'm already working 9 hours a day and doing other stuff too, adding other 5 hours on top of it is simply not going to be possible, so on my case (and I'm guessing is the case of many others) I'm not really worried of consuming more than actual 48 hours of work here.
