That's the idea. There will be quite a few timed events that will require your input and the world will change depending on your action. This is already in the game in some way.
For example, more kobold groups will start appearing over the map as time passes. If you start killing them, then even more will appear.
Or the cure quest. If you finish it, the trader will survive and you will get a discount. But you can also completely ignore it. If you do so, the trader will die and he will be gone forever.
I have many ideas for timed quests or events, but I want to make the world bigger before I start working on them.

Another question, will having visible mutation do anything to my encounter later with Mr White? The bonuses with having mutation seems like not a bad idea but with some people not liking mutation I was thinking if Mr White was the same as those people who hates mutants
Edit: Also will my role as cult initiate effect any encounter? or is the role stuff still under development?
White doesn't care about mutations at all. He will even encourage you to get them in the future.
I think there is one encounter that gets affected by being initiate, but I don't remember the details. Other than that, the rank itself does nothing. It's the uniform that matters.
Wearing the uniform will make pretty much everyone offer you a discount when trading. It will also make Billy talk to you if angered him before.
