it happens if you fell under the map. 

Specifically it happens when you, spoilers, glitch out of bounds. I accidentally discovered it while climbing in the vents and managed to clip through the ceiling by vaulting. 

pretty sure it's a very rare chance to randomly happen, otherwise doing weird maneuvers or messing around out of bounds can cause you to clip to there

specifically clipping into walls and floors seems to trigger it, as in being still stuck in them when the game checks. there's many ways to accidently fully clip through things and the game won't boot you to the backrooms for it
