for me it happened when i sent kerfur out to do reports, he went towards the lake (presumably towards oscar or something) and i heard a placeholder sound (kleiner saying fiddlesticks) and i couldn't pause for a while, i checked kerfur's camera and he was in the flesh pit
i tried to retrace his steps and found a blood stain, i assume that's where he disappeared
it's been a few days and i haven't found any sign of him, i've heard he has a range that he could come back from but i'm not sure how much of a range it is
Have you checked each ping personally? I know it may sound redundant, but I did see someone suggest to check out your windows, so checking the pings close to your base might be worth looking into, as well. Especially when things on the map appear close to your base, it's hard to determine what's new, what was there, and what wasn't.
