Hey, this is a fun concept! I think some of the colors should've been different, as they were sometimes hard to see or hurt my eyes on the background (for example, the dark blue I had to squint to see). The buttons on the main menu and how to play screens were a little small and I would've liked them to be a bit bigger. Also, the "how to play" screen was filled mostly with text and information that I looked at once, knew I wouldn't remember, and decided I'd just figure it out as I played. 

In the actual game, I think the shooting elements and dodging enemies was really fun. Changing shapes and shooting types added variety for different types of play. I didn't like the one with no shooting and no moving, as I thought doing nothing was kind of boring. But other than that, I enjoyed the rest of the experience. Well done :)

Some background could be a good addition as I didn't understand I was moving at first.

I agree that the ‘next’ box is cool to plan ahead. That is, if you can remember all the shapes. Especially at the beginning, because of this and the solid color background I was not able to tell whether I’m moving or not or what is happening which didn’t feel good. Edit: forgot to mention that I loved that the music changes with the shapes

I like how there is a 'next' box. That small detail really opens up the game.

I wonder what would happen if the shape of the player and the shape of (all of the) opponents could change independently (giving two 'next' boxes). That'd give a whole lot more combinations!

I made it to wave 5! I really liked how the controls constantly change, none of the control schemes felt bad or frustrating.

The art style was pretty simple but carried the game design well. Nice job!!

I like how the enemies swap with you as well. It makes it so you can plan for the next shift accordingly. The biggest problem to me was that the background is one solid color. This makes it hard to gauge how fast you are going and where you are especially when there aren't a lot of bullets on screen. This also caused the forced movement shapes to be pretty disorienting. Besides that the next biggest problem would be the lack of audio feedback for when you get hit. It can be hard to check your health while playing at the same time.
